Membership - Declaration formI, agree and declare that as a PTCA member, I must strive to be the best coaching professional I can and also fulfill the obligations of being an upstanding citizen and role model to my player, to my community, and to the tennis coaching community at large. I will maintain and observe a sound base of moral integrity and to exemplify the highest ideals of our profession.
I will be fair, honest, and sportsmanlike at all times, conduct myself in a professional manner both on and off the court and exemplify the highest standards of moral character. I will meet all financial responsibilities promptly and honor all contracts. I will continue to enhance my professional goals by continuing educational pursuits, seminars, conventions, etc., and striving for excellence through professional development.
My personal grooming habits should be of the highest standard. I will respect each place I work, play or visit so that I am in accordance with their standards as well. I will coach and abide by the rules of the game in letter and in spirit and always respect officials and their decisions. I will not give lessons or solicit coaching where a resident PTCA- professional is employed, unless prior approval has been given. I will encourage healthy respect for my fellow colleagues and PTCA.
I further agree, that Subject to my application being successful, I can refer to myself as a PTCA Coach (Registered, Certified or Master) for the duration of my certification. • I acknowledge that I am obliged to continue to complete professional development activities and the relevant requirements during this three year period in order to renew my certification 6 months before expire date • I agree to maintain a current first aid qualification and adequate insurance, and to provide PTCA with evidence if requested from time to time. • I agree to abide by, be bound by and encourage compliance with PTCA Member Protection By-law, anti-doping policy, events discipline policy, general code of conduct, coaches code of conductand any other relevant policy(s) which is made known to me, as amended from time to time and available for viewing at www.ptcatennis.org. • I acknowledge that disciplinary action may be taken against me (including suspension or revocation of my certification) if I commit an offence under the PTCA Member Protection By-law. • I acknowledge that I am ineligible to work or seek work as a tennis coach if I am a prohibited person as defined by the PTCA Member Protection By-law. • I confirm that I have completed a Prohibited Person Declaration as part of my application for coach membership. • I agree and acknowledge that it is my responsibility to comply with and submit to any relevant international screening requirements imposed on people working with youth and children, and to provide PTCA with evidence of a national police certificate as required. • I will maintain high professional standards at all times and not bring PTCA, its Member Associations, the tennis coaching profession or the sport of tennis into disrepute. • I confirm that all information provided by me in this PTCA membership application is true and correct.

Prohibited Person Declaration
Membership – Prohibited Person Declaration Herewith I do accept that the PTCA Member Protection By-law makes it an offence for a Prohibited Person (defined as a person who has been convicted of a serious sex offence) to work or seek work in the following roles:1. coaches who are appointed or seeking appointment (whether employed, contracted or otherwise) for reward;2. volunteer coaches appointed or seeking appointment, who will or are likely to travel away with teams of players under 18 years of age; and persons appointed or seeking appointment to a role in which that person is likely to have individual and unsupervised contact with players under 18 years of age (for example, a team manager).The PTCA Member Protection By-law also makes it an offence to appoint, or continue to appoint, a person to a role set out above: without first obtaining this declaration; or where this declaration reveals the person is a Prohibited Person.The PTCA Member Protection By-Law defines a serious sex offence to mean an offence involving sexualactivity or acts of indecency including but not limited to:Rape, Indecent assault, Sexual assault, Assault with intent to have sexual intercourse, Incest, Sexual penetration of child under the age of 16, Indecent act with child under the age of 16, Sexual relationship with child under the age of 16, Sexual offences against people with impaired mental functioning, Abduction and detention, Procuring sexual penetration by threats or fraud, Procuring sexual penetration of child under the age of 16, Bestiality, Soliciting acts of sexual penetration or indecent acts, Promoting or engaging in acts of child prostitution, Obtaining benefits from child prostitution, Possession of child pornography, Publishing child pornography and indecent articles DeclarationI am aware that I am ineligible to work or seek work in the roles set out above if I have been convicted of a serious sex offence, as defined in the PTCA Member Protection By-law.
